Parental Consent Form for Minors

Dear Parent or Legal Guardian (“Parent”): Please provide the information below so that we may evaluate whether your minor child (“Minor”) may attend Califur 2010; A Sci-Fi/Fantasy Convention and Trade Show owned/operated by Fennec Adventures.

Parent may designate and authorize an alternate adult who may be a spouse or an unrelated person over age 21 to act as chaperone or alternate contact in case Parent is not reachable at above contacts:

Parent hereby grants permission for the above named Minor to attend as a guest at Califur 2010, held at the Irvine Marriott, in Irvine California no earlier than June 4th, and no later than June 6th.

 

Parent understands and agrees to the following Agreement on behalf of Parent and Minor and must sign and date this Agreement on the last page.

 

1.            Eligible Minor

A Minor is defined as a person under the age of 18. All minors must have their Parent complete this Agreement and mail to Fennec Adventures prior to attendance, or have Minor present this letter at the time they pick up their registration. All minors without parental consent will be refused admittance. Minors under sixteen (16) years of age are not eligible to attend Califur 2010 unless accompanied full time by Parent.

 

2.            Califur 2010 is NOT supervised.

While we will have security staff to assist anyone during the convention, the public areas of the Irvine Marriott are unsupervised. Fennec Adventures does not position or market its services to minors, we simply provide attendance. No special provisions or amenities or supervision for Minor participants are provided.

 

3.            Fennec Adventures Does Not Act in loco parentis

Fennec Adventures does not and will not act in loco parentis (“in the place of a parent”) with respect to a Minor while on convention premises or during Califur 2010. This means that Parent is entirely responsible for the care of the Minor at all times, even if Parent decides not to accompany or remain with Minor during Califur 2010. Fennec Adventure’s duty of care for a Minor participant shall be no greater than for an adult participant.

 

4.            Parent or Chaperone May Accompany Minor

Parent or chaperone may accompany Minor to Califur 2010 may allow Minor (over 16) to attend unaccompanied. Fennec Adventures requires Minor less than 16 years of age to be accompanied by Parent or chaperone during Califur 2010. Upon request Califur 2010 will provide admission for Parent or designated chaperone at a reduced rate to accompany Minor during Califur 2010. The guardian must register with the Minor at the time the Minor picks up his/her badge. If a choice of full attendance is desired please arrive at least 1 hour early.

 

5.            Parent Responsible for Conduct, Care, Supervision and Safety of Minor

Parent is solely responsible for the conduct, behavior, care, supervision and safety of Minor while Minor is in attendance at Califur 2010, whether or not Parent or chaperone accompanies Minor. Califur 2010 offers live computer equipment equipped with unrestricted access to the Internet. Parent is solely responsible for Minor's use of the Internet during Califur 2010.

 

6.            Right of Dismissal

Fennec Adventures reserves the right to dismiss a participant from any Califur 2010 event if the participant in Fennec Adventure’s sole judgment acts inappropriately or disruptively. In case of such dismissal, Fennec Adventures’ will have no obligation towards the refunding of any fees paid by participant or participant's guardian in respect of the convention and Fennec Adventures shall not be responsible for any consequential loss or inconvenience arising whatsoever.

 

In case of dismissal of a Participant who is a Minor unaccompanied by a Parent, Fennec Adventures, as a courtesy, will make reasonable efforts to contact the Parent and chaperone/alternate adult at the contact numbers supplied by Parent, and notify of the dismissal of Minor.

 

7.            Release

I, on behalf of myself, my child, our heirs, executors, administrators, successor and assigns, hereby release and discharge Fennec Adventures, its officers, directors, agents, and employees with respect to all activities associated with, or any damages arising out of Califur 2010 and from any cause of action of any nature whatsoever arising from my child’s participation in Califur 2010.

 

Understood and agreed: On behalf of myself and my Minor child I hereby certify that I am the Parent or legal guardian of the Minor named above and that I understand and agree to all the provisions above as a condition of my Minor child registering and attending Califur 2010 and/or all Fennec Adventures activities offered during this event:
